Here's the bustout story as Dave Fox told it. He started the hand well off at 116,000 in chips. Intending to raise to 7,500, Fox slipped and threw out an extra gray chip, making it 12,500. Pat Madden in seat one asked him if he intended to do that. "I'll tell you after the hand," Fox said. "All right, I call," said Madden. The flop was {8-Hearts}{6-Hearts}{6-Diamonds}. Madden checked. Fox made it 17,000 and Madden check-raised to 40,000. Fox went all in and Madden called.

Fox showed {9-Hearts}{7-Hearts} for the straight draw, flush draw and straight flush draw. Madden had {A-Hearts}{A-Clubs}. The turn and river were {2-Diamonds},{K-Diamonds}, and Fox was out.
